Why is my Transit losing coolant?

I own a Ford Transit (2000 model). Recently, I've noticed white smoke and a loss of coolant. In fact, all of the coolant has gone, even though the van shows no signs of overheating. Any idea what the fault is?

The most obvious reason is a cracked cylinder head or failed cylinder head gasket.
